A child fell off of a climbing tire while playing in the outdoor play area. The area surrounding the tire did not have sufficient ground cover to prevent injuries. The licensor observed approximately 3 inches of loose play chips near the area where the child was injured. The ground cover requirement is 9 inches. ?? "No observation provided."
